Eglin Fitness & Sports Reservation Form

Please complete and submit the form below to request a reservation of Eglin Fitness & Sports facilities and equipment.

PLEASE NOTE: This form generates a REQUEST only; it DOES NOT secure a reservation. Following a Requester’s submission, a member of the Fitness & Sports staff will contact the Requester with approval (or disapproval) of their reservation request. No reservation is confirmed without this expressed approval.

Read and Acknowledge

1. I have read and understand the Eglin Fitness & Sports Center Reservation Policy.

2. I understand I am liable for any damage incurred to property, facility, or equipment while in my possession.

3. I will contact a Fitness Center staff member immediately if I have any concerns regarding my reservation.

4. Reservations are subject to change or cancellation as mission requires at any time at the discretion of the fitness center director. A staff member will make every attempt to notify the requester as soon as possible, should this occur.

5. The using unit/organization is responsible for set-up, tear-down, and cleaning of the area, including trash removal during use of outdoor facilities. Non-compliance will result in disapproval of further unit/organization reservation requests.

6. Reserved courts or fields will return to an available/open status in the event of a no-show (15 minutes after reserved time).

7. All sports equipment for outside use must be identified prior to the event and will require POC to sign an AF 1297 Temporary Hand Receipt.

8. If food is going to be cooked and/or served at an FSS facility, the reserving unit/organization must notify Public Health at 850-883-0308 or -8608, prior to the day of their reservation.

9. All participants who are utilizing the main Fitness Center must either have 24-hour access or sign into our guest sheet. Letting unauthorized patrons in through any door is prohibited.
